- On the new page, you’ll need to enter your registered mobile number and solve a captcha.
- Once done, verify your number with the received OTP (one-time password).
- After verifying the OTP, you’ll be directed to a new page.
- Here, fill out the complete Ayushman Bharat Yojana Card Application Form.
- Provide accurate information, including your name, address, and other necessary details.
- Check all the details, and then submit.
- Once submitted, your Ayushman Bharat Yojana Card Application Number will appear on the screen. Note it down safely for future reference. The application details will be sent to your registered mobile number for your records as well.
- Now, the verification process will begin, involving the confirmation of your application.
